Policies Agreement
(MUST E-SIGN ONLINE TO BE ELIGIBLE TO PARTICIPATE AS OF UPDATED POLICIES 9/1/23)
Policy Length and terms (REGULAR SEASON ONLY, SEPT-MAY)
The signing of this policy is based on a 9 month season term agreement. A 30 day notice must be given if canceling participation from class. If a 30 day notice is not given there will be a termination fee equal to 30 days participation fee due plus any outstanding balance owed. No cancellation after March 1st if confirmed for Annual Recital. Remaining balance for the season (March/April/May) is due if cancellation is submitted after March 1st. Withdrawal from any class (one or all) must be in writing to the owner, Lacy Draznik.
Family Discount
$5 off monthly tuition for each sibling.
Enrollment Fee (REGULAR SEASON ONLY, SEPT-MAY)
An annual non-refundable Enrollment fee is required for all students wanting to participate. $25 per student

Tuition and Payment (REGULAR SEASON ONLY, SEPT-MAY)
Automatic payments are strongly encouraged and appreciated. The customer must “opt in” for this processing in your Parent Portal account under “edit payment information”. This is a FREE service (no processing fee). Auto-tuition will be drafted the 1st of each month. A 3% processing fee will be added if opting out of auto-tuition with CC. We now have FREE ACH (checking) Payment available! Reminders are sent out prior to the first of each month, and a receipt will be emailed out at time of payment. Please make sure to update your card information if any changes occur by end of month. A $15 late fee will be added if payment is not submitted by 15th of each month. Any student that is 30 days past due tuition is subject to termination until balance is resolved.
Returned check fee is $30.

Default of payment
Should you default on any payment as called for in this policy, MCDM will have the right to declare the entire remaining balance for season, all costs of collection, agency fees, court costs, and/or attorney fees.
Please note: MCDM does not mail out monthly invoices for tuition payments. You may at any time contact the owner to request an invoice, review your balance, and/or posted charges. A families account is also viewable through your online “parent portal” at any time.
Paid in Full Discount

Dance Recital and Costume Fees (REGULAR SEASON ONLY, SEPT-MAY)
Costume fees for the year-end dance recital will include one pair of tights ($80 including sales tax and one pair of tights for child sizes, $90 including sales tax and one pair of tights for XL child to adult sizes, pricing as of 2/1/2023). Payments are split between November 1st and December 1st tuition payments (half and half). A $15 late fee per month will be added if payment of costume is not in by due date unless other arrangements have been discussed. No costume fees will be refunded after December 24th. The purchase of Recital costume confirms Recital participation. Recital costumes are not eligible for return if policy is terminated after December 24th unless approved by Owner, Miss Lacy.
Recital fee is $60 per family. The first half is due with March Tuition ($30 processed with auto pay on March 1st) and the second half is due with April Tuition ($30 processed with auto pay on April 1st) . Recital Fee includes 1 complimentary ticket to Recital AND the professional digital video recording of Recital. A $15 late fee will be added if Recital Fee is not in by April 15th unless other arrangements have been discussed. Recital Fee includes production and venue fees. The payment of Recital Fee confirms Recital participation and is not eligible for refund after March 1st unless approved by Owner, Miss Lacy.
Recital tickets
Each Family receives 1 Complimentary ticket with Recital Fee. Recital tickets are $12 each online prior to Recital. Ages 3 and under that can sit on a lap are free. Tickets will be $15 at the door with cash, check, or cc. Online ticket sales will open on April 1st. Link will be emailed out and on our Recital page. (2/29/24)
Attendance
Students are expected to attend all scheduled classes to ensure students’ progress. Please email any absences as soon as possible, or within 24 hrs prior to Miss Lacy (lacy@mcdancemovment.com). Students who miss a class need to attend a make up class similar in style and level. Please notify and/or approve make up class with Miss Lacy. A private lesson will need to be scheduled if any recital choreography is missed. If a recital participant has more than 4 classes unexcused absences, the studio reserves the right to deny the student of recital participation.
MCDM requires all staff and students to stay home when they are ill. In order to reduce community spread of diseases, we follow best practices and recommendations from our local health department and county guidelines. MCDM understands that unlike older children and adults, young children cannot be expected to maintain social distancing at all times.
All studio classes have the option of being held online at any time due to quarantine. In the event a class is unable to be held at the studio, classes will convert to an online learning platform until in-studio classes can resume.
COVID POLICY
I understand that while MCDM has undertaken reasonable steps to lessen the risk of transmission of COVID-19, MCDM is not responsible in any manner for any risks related to COVID-19.
I will keep my child home if they have had any flu like symptoms in the last 24 hours or have been in contact with anyone who has been confirmed with Covid in the last 24 hours.
No refunds will be issued once classes have started for that month due to any Covid-19 related issues.
Attire
MCDM maintains a dress code to encourage concentration and a sense of inclusiveness. Uniformity in dance wear allows the teachers to assess how well the students are implementing the technique being taught, problems with alignment, and other important aspects of dance training. Students must comply with class attire requirements specified by the teacher for each class. Please see “Dress code” page here.
Class Behavior and Guidelines
Students will treat all teachers, fellow dancers, and themselves with respect. Dance class etiquette is instructed and advised. No gum in class. Please try to use bathroom before or after class. Water bottle only in class, no food or drink. Food and drinks other than water are permitted in the lobby only please. Please have shoes on and hair secure before class begins.
NO REFUNDS WILL BE GIVEN FOR STUDENTS MISSING CLASS, CLASSES CANCELLED DUE TO INCLEMENT WEATHER, MISSED CLASSES DUE TO VACATION, HOLIDAYS, PANDEMIC, AND/OR DAYS OFF. PAYMENT CAN BE PAUSED OR PRO RATED IF SERIOUS INJURY OCCURS AND MUST BE FOLLOWED BY A DOCTORS NOTE INDICATING HOW LONG CHILD WILL BE UNABLE TO PARTICIPATE.
Parent/Guardian/Adult Student Responsibility to be Aware of Dates, Events, and Related Activities
It is the responsibility of the parent, guardian, or adult student to be aware of all school activities such as: rehearsals, recitals, extra classes, and dates the school is open and closed. It is the parent’s or adult student’s responsibility to regularly check emails, studio social media, and/or studio website to ensure they are informed. It is also the responsibility of the parent, guardian, or adult student to inform the school of any address, phone, or email changes.
Photo Release
The school is hereby granted permission to take photographs and video of the students to use in brochures, web sites, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.
Waiver and Release of All Claims/Parent Consent
Although every effort is made to ensure the safety of my child, I realize there is always risk of accident. If necessary, I authorize any employee, director, or volunteer of MCDM to administer first aid and/or emergency treatment (call 911) for my child on my behalf. I further release MCDM from all liability of injuries or damages arising out of personal injury of any kind. This applies to any time in the studio or at a MCDM event. I acknowledge and understand that MCDM will attempt to first notify parents and guardians in case of an emergency.

I have read Miami County Dance Movement policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Miami County Dance Movement does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Miami County Dance Movement, and I voluntarily assume the risks associated with such participation.
